Here at Alliance Primary Care: Danielle Hawkins, APRN, FNP we are always accepting new patients. Regardless of whether you are sick or are simply due for a regular check-up, we are happy to welcome you to our office. We know going to the doctor can be a little overwhelming at times, so we are happy to provide you with our new patient forms ahead of time so that you can fill them out prior to your appointment and save you time during your visit.

Remember to bring a list of any medications you may currently be taking as well as any previous medical records you may want to provide. New Patient Forms

HMO Insurances:
If you have an HMO insurance check your insurance card to make sure Danielle Hawkins is your assigned PCP. If she is not your assigned PCP we will ask you to contact your insurance to have her assigned prior to booking your appointment.

Please complete new patient check in paperwork online prior to your appointment; if you are unable to do so please plan on arriving at least 15min prior to your appointment to complete this.

Make sure to upload a copy of your insurance card and drivers license or bring them with you to your appointment.

Special Forms Such as FMLA:
We charge a $40 fee for any special paperwork needed. Paperwork will be completed within 1 week. We do not do FMLA for conditions we are not actively treating.
